Level 4 builds on terminology introduced in Levels 1–3 while adding more advanced vocabulary related to the uterus, uterine tubes, ovaries, testes, spinal cord, bone marrow, cartilage, ribs, paralysis, weakness or partial paralysis, breathing, tumors or masses, stopping or controlling, absence, opposition, and new formation.

Students are challenged to distinguish closely related terminology such as -plegia vs. -paresis, -oma vs. -megaly, -stasis vs. -stomy, and a-/an- vs. anti-, while also recognizing that some combining forms—such as myel/o—may have more than one accepted meaning and require sufficient context for precise interpretation.
